Impacto de la receta electrónica en la unidad básica de atención médico/a-enfermera/o en Atención Primaria

Resumo

Objective. The coordination of physician/nurse consultation is a basic factor of primary care. This paper analyzes the impact of the implementation of the electronic prescription on the functioning of physician and nurse consultations. Material and methods. A cross-sectional retrospective study was carried out from February 2015 to January 31, 2016 without electronic prescription and from February 2016 to January 31, 2017 with the introduction of the electronic prescription. We analyzed the increase and reduction of consultations, the affected type of consultations and the impact in the patients attending time. Results. A total of 7,006 nursing consultations were studied in 2015 and 4,612 in 2016. The nursing consultation suffered a total reduction of 34.18% of consultations with a similar reduction of consultation on demand and concerted consultations. In case of the medical consultation, 7,586 consultations were studied in 2015 and 8,622 in 2016, which means an increase of consultations of 13.65%. In the year 2015 the administrative consultation represented 15.42% of the total demand for the doctor’s consultation, while in 2016 the administrative consultation represented 29.24% of the total demand. As for the times for consultation, in 2015 the Doctor spent 10.54 minutes per consultation on demand and in 2016 spent 8.72 minutes, a reduction of 1.82 minutes of patient care loss. Conclusion. The introduction of the electronic prescription has meant a significant increase in the healthcare pressure of the physician’s consultation with the consequent impact on the patient care time. At the same time, it causes a migration of the patients from the nursing consultation leaving the nursing activity very diminished.
