Resumen

The fact that an infectious agent can initiate chronic inflammation and constitute the essential motive in the transformation of B lymphocytes and subsequent lymphomagenesis has regained interest in the scientific community in recent years. We present a Clinical Case in order to reflect the need for awareness in the consideration of Primary Cutaneous B-Lymphocyte Lymphoma of the Marginal Zone (PCMZL) as a possible diagnosis within indolent lesions of torpid evolution, with good prognosis, and that currently have shown a higher incidence in children and adolescents. The patient had a history of recurrent pharyngotomsilar infection with a strong association with the development of PCMZL.It is possible, just as the association of the etiopathogenesis in some cases of PCMZL with infection by Borrelia Burgdorferi and other bacterial infections has been shown, that chronic infection by Streptococcus Pyogenes could induce the appearance of a transformation of the B lymphocytes of the marginal zone into a neoplastic cell, thus developing marginal zone B-cell lymphoma. This process of chronic aggression by an antigen (streptolysin O), would produce a monoclonal selection of plasma cells that would be responsible for the production of Ig/streptolysin O, this monoclonal lineage would undergo a neoplastic transformation throughout this sustained stimulation, managing to escape to regulatory mechanisms, and maintaining continuously high ASLO titles, as in the case we present.
