Introduction. Ovarian carcinosarcomas are approximately the 3% of the ovarian tumors. Material and Methods. Woman of 30 years old who is diagnosed of a huge pelvic mass which seems to be a cystadenocarcinoma located in the ovarian. A laparotomy is done to the patient. Pathological Anatomy: mucinous cystadenocarcinoma in left ovarian. pT1a. The patient is operated on again. Result: metastasis of carcinoma in paraaortic and pelvic lymph nodes. After one week our patient is diagnosed of paralyticileus and she is re-operated on. 40 cm of intestine are resected and they result to be infiltrated by carcinosarcoma. Peritoneal fluid: postive to malignant cells. Results. After being in ICU for one week, our pacient suffers a multi-organic failure and she dies just three months after the first surgery. Conclusions. The ovarian carcinosarcoma used to be diagnosed in an advanced stadium because of its few symptoms. Some studies consider the initial stadium of the disease as the only prognostic factor. The treatment is controversial. It is accepted that the best option is to achieve a complete cytoreduction, which seems to improve survival. It is needed to do multicentric studies and to introduce new therapies to improve the prognostic of all these patients.
